Fever in a child
Fever is one of the most common reasons parents visit a pediatrician. It is important to understand exactly what we mean and whether it is cause for serious concern.
Fever is an elevated body temperature above the normal range, which is usually 36°C to 37°C. In children, fever is considered to be:
- 38°C when measured under the armpit or on the forehead.
- 38.5°C when measured rectally.
How to measure a child’s temperature?
There are several ways to measure a child’s temperature:
- Electronic thermometer: Can be used under the armpit, orally (for older children), or rectally (for infants and young children). Rectal measurement is considered the most accurate for infants and young children.
- Non-contact thermometer (forehead): Convenient and quick, but may be slightly less accurate than an electronic thermometer. Follow the manufacturer’s instructions.
- Ear thermometer: Measures the temperature in the ear and is quick but requires proper technique.
How to manage a child’s fever at home?
- Ensure the child rests.
- Maintain proper hydration. Give small amounts of fluids frequently (water, tea, electrolyte solutions).
- Reduce fever with antipyretic medications containing paracetamol or ibuprofen at a dose appropriate for the child’s weight (according to the leaflet or doctor’s recommendation).

You may use physical cooling methods, such as:
– Lukewarm (not cold!) compresses on the forehead, neck, armpits, and groin.
– Lukewarm bath (water 1–2°C lower than the child’s body temperature) for a few minutes. - Do not overheat the child. Dress them in light, breathable clothing. Maintain room temperature around 20–22°C and ventilate regularly.
- Monitor the child’s condition and measure temperature every few hours. Pay attention to other symptoms.
Dosage of antipyretic medications
- Paracetamol – 15 mg/kg every 4–6 hours, maximum 4 doses per day.
- Ibuprofen – 10 mg/kg every 6–8 hours, maximum 3 times per day. Note: not for children under 3 months.
- Additionally, a doctor may prescribe prescription drops containing metamizole.
When does a child’s fever definitely require a doctor’s visit?
- Child under 3 months old with temperature above 38°C.
- Fever accompanied by other concerning symptoms, such as:
– Lethargy, excessive sleepiness, or difficulty waking the child.
– Irritability, difficulty calming the child.
– Lack of appetite or refusal to drink fluids, dehydration signs (infrequent urination, dry mouth and tongue, no tears when crying).
– Vomiting or diarrhea.
– Rash
– Headache
– Difficulty breathing or rapid breathing
– Seizures
– Severe abdominal pain - Very high fever (above 39°C) that does not respond to antipyretics.
- Child has chronic illnesses.
- And most importantly – the child’s condition causes serious concern for the parent!

Transfontanelle ultrasound (head scan) + consultation

Transfontanelle ultrasound allows:
- assessment of brain structure development and detection of congenital abnormalities,
- diagnosis of intracranial bleeding and birth-related injuries,
- monitoring of ventricular size and detection of hydrocephalus,
- evaluation of cranial suture closure,
- diagnosis of post-traumatic changes, such as skull fractures,
- vascular flow assessment using Doppler ultrasound to evaluate brain blood supply.
Transfontanelle ultrasound is particularly recommended for premature babies, newborns from high-risk pregnancies, and infants with concerning neurological symptoms. It is also indicated in cases of developmental abnormalities or irregularities detected during prenatal screening.
How is this examination performed?
During the examination, the doctor places a special ultrasound probe on the baby's fontanelle. The resulting image allows for a detailed analysis of brain structures. The test is painless, non-invasive, lasts only a few minutes, and can be performed in the presence of parents.

- diagnosis and monitoring of both acute (e.g., bronchitis, pneumonia) and chronic respiratory conditions,
- assessment of congenital lung abnormalities.
Imaging diagnostics can detect diseases at an early stage and monitor treatment effectiveness. Pediatric lung ultrasound is commonly performed in cases of prolonged unexplained cough, recurrent respiratory infections, or suspected pneumonia.
How is this examination performed?
At Pozmedica Clinic, pediatric lung ultrasounds are conducted based on a referral from the attending physician. The procedure is painless and requires no special preparation. After applying a special ultrasound gel to the child’s chest, the doctor moves the probe while observing the images on the monitor.
When is lung ultrasound recommended?
The examination is performed on a physician’s order when it is necessary to detect lung changes that are not visible during auscultation. Ultrasound allows visualization of soft tissue structures and identification of possible abnormalities in the lungs.
